Student Medical and Emergency Information — 2026 source edition
I, the undersigned parent/guardian, give consent for Goroka Grammar School staff to take all reasonable steps to ensure the health and safety of my child in the event of illness or injury.
I authorise the permissions I selected:
- Administration of first aid treatment by trained school staff.
- Medical or hospital treatment as deemed necessary in an emergency.
- Use of an ambulance should the situation require it.
- Administration of prescribed or non-prescribed medication if needed and approved by the parent/guardian.
I understand that the school will make every effort to contact me or the emergency contact listed before taking further medical action, except where immediate treatment is required to prevent serious harm.
This information will be stored securely and accessed only by authorised school personnel in accordance with Goroka Grammar School’s Health & Safety and Privacy Policies. The confidential record is kept with the School Nurse and Administration Office.
Specific medication instructions — Primary Handbook source
Students are not allowed to carry medication. Hand medicines to the class teacher for Primary, or the school nurse or matron for Secondary and Boarding. Provide clear instructions from a parent or doctor. The parent supplies the medicine, dose instructions, timing, applicable dates and instruction source. The website does not prescribe or calculate doses. Specific authorisation is separate from general emergency permissions. Submission does not approve administration. Staff review remains separate. Changes or cancellation create a new record. Earlier instructions are retained. Staff review the instructions before giving medication.
